There are no standards in Latin America. We always follow the manufacturers protocol during our first test. We have tested different protocols of antigen retrieval in our breast cancer panel, and we always test different protocols for each antibody in our laboratory. Therefore, an antibody to be used in diagnosis receives a special attention in order to give the most reliable protocol. In summary, if the antibody belongs to a kit, we will strictly follow the manufacturers instructions. If it does not, we standardize to our conditions. Connection: How would you rate European (UK NEQAS, NordiQC), the US and Canadian IHC Standards? All of them are interesting ways to evaluate immunohistochemistry laboratorie in their respective countries. Connection: You use ACIS® III Image Analysis System. What constitutes standardization of image analysis as applied to Immunohistochemistry (IHC)? Can we have a standardized procedure to interpret IHC assays around the world? We use ACIS® III in our research projects and in routine for ER/PR, c-erbB-2 and Ki-67 index evaluations. However, to use image analysis systems in diagnostic procedures in Latin America is very difficult because it costs money, it is time consuming and very few centers have specialized pathologists. We need more scientific work in order to translate our results into something that will benefit the patient. Connection: Why is standardization of image analysis in diagnostic pathology important? A diagnostic or prognostic procedure is essential in medical practice. Standardization of image analysis in diagnostic pathology is important because it is not influenced by subjective analysis. Introduction of an objective analysis by automated systems will be a major plus in patients care. Connection: What are your recommendations for standardizing image analysis in an anatomic pathology laboratory? The main recommendation is to know the process very well. In addition, continuous education of all laboratory personnel regarding the importance of standardization procedures is equally important. Connection: Is a universal image analysis system feasible? It is a matter of time and cost. It is feasible and necessary, but we have a long way to go. Connection: In your opinion, what is the biggest hurdle for standardization in image analysis? Time and cost. Connection: With image analysis in mind, what is the minimum information needed to enable the interpretation of IHC results? The pathologist has to understand the algorithms and the principles of optical density analysis. Connection: Is there a lot of variability in manual IHC testing? How does ACIS® III Image Analysis System improve the scoring of IHC slides? Yes, there is a lot of variability in manual IHC testing. Protocols that take in considerable numbers of stained cells and intensity of the reaction have very poor reproducibility and are very subjective. Image analysis can improve dramatically how we score our slides. We still have some difficulties in comparing results, but it will be better when we can compare two objective scores. ACIS® III Image Analysis System improves the scoring of IHC slides by giving an objective analysis instead of a subjective analysis. It also improves the interobserver reproducibility by permitting better check and comparison. Connection: Where do you see IHC in 10 years? Use of more standardized kits, personalized treatment and increased utilization of prognostic tests. Connection: Living in Sao Paulo, Brazil, how have you kept up with the changes occurring in IHC/Pathology? I pay attention to key publications in the IHC/Pathology field, attend important scientific meetings such as USCAP/IAP/ESP and visit main diagnostic centers.
