Kumar, PhD Managing Editor Welcome to another informative issue of Connection, our special issue devoted to IHC Standardization Around the World This issue . includes opinions, articles and questions and answers from IHC experts in Australia, Brazil, Canada, China, Denmark, India, Japan, the United Kingdom and the United States. Thanks to the advent of modern communication technology, this entire report was compiled remotely by electronic communications and telephone conversations with the experts. In brief, this issue highlights the challenges that lie ahead in standardizing IHC around the world. For instance, the technical problems that developed countries (Australia, Canada, Denmark, Japan, the United Kingdom and the United States) and developing countries (China, Brazil and India) are facing are nearly the same. Most laboratories around the world have no control over the pre-analytical factors, i.e. mainly tissue handling following surgery and prior to submission to the laboratory. In a similar manner, one of the analytical variables, i.e. duration of tissue exposure to the fixative, seems to vary a lot between laboratories in all these countries. However, the existing problems in developing countries are different from those in developed countries in terms of basic education in laboratory practice and infrastructure as well as in terms of quantity of samples received by pathologists. In India, the allopathic doctor-population ratio is about 1:1700 (1) and the number of patients attended by a physician in a rural area can be as high as 300 per day (2). This results in enormous workloads to all medical professionals including pathologists leading to quality control issues associated with patient samples. In Brazil, the doctor-population ratio is higher, 1:900, but the problem here stems from the lack of adequate laboratory training and other associated costs. There are other issues too: The lack of funds to buy the most basic equipment (think of Africa), expenses associated with visiting state-of-the-art diagnostic centers in the developed world and last, but not least, no organizational structure provided by orginazations such as e.g. USCAP, UK NEQAS or NordiQC that can promote standardization. All this leads to one rhetorical question: Can standardization of IHC testing be achieved? The answer is an unqualified Yes But unless . every country works toward developing its own national standardization organization and take part in an international meeting represented by the heads of such organizations (such as the Intergovernmental Panel on Climate Change, or IPCC meetings), standardization around the world is not possible. The challenge for all of us is to act in a serious, yet timely fashion given the liabilities that exist for the patient. While uncertainties remain, we can at least begin to address the factors that contribute to a consensus in this regard. I welcome debate and suggestions. 1.
